The Referral & Consultation Process

Initial Referral & Consultation

Referral partners begin with a brief consultation to discuss presenting concerns, referral questions, case context, and service needs. This helps determine appropriate scope, expectations, and next steps.

Assessment, Support, or Consultation Services

Following referral, services are tailored to identified needs and may include clinical assessment, consultation, record review, psychoeducation, stabilization-focused support, behavioral insight, and practical recommendations.

Clinical Summary & Ongoing Collaboration

When appropriate and authorized, clinically grounded feedback, summaries, recommendations, or consultation insights are provided to support understanding, care coordination, informed decision-making, and next-step planning.

Services are collaborative, clinically grounded, and tailored to referral needs, with appropriate consent, privacy, and ethical considerations guiding communication and information sharing.

Documentation & Information That May Be Requested

To support a thorough and efficient consultation, the following information may be requested when relevant to the referral and available with appropriate authorization:

Case-related Documentation: Legal records (if applicable)

•

History: Educational, behavioral, psychosocial, and functional history

•

Evaluation Summaries: Prior clinical assessments and behavioral health summaries

Case Goals: Specific referral questions or consultation goals via questionnaire
